ABOUT THE FACE
Your
face is an essential part of your identity. Usually, it is the first
thing people notice about you. Your face conveys expressions,
feelings and moods. Unfortunately, there may be a facial feature
that seems out of place to you; or your face may display the effects
of the natural aging process. Dr. Lichten understands your concerns
and he specializes in a variety of facial cosmetic and
reconstructive surgical procedures that can enhance your appearance
and reverse the effects of aging. You can improve your outward
appearance and, in turn, restore the confidence that comes with it.
When choosing a surgeon, it is important to select a doctor who is
well trained in his field. Dr. Lichten can assist, for example, in
creating a nose that fits in with your other features and looks
natural, or he may be able to eliminate wrinkles and drooping
eyelids that appear as you age. He will also explain to you in
detail the benefits and the limitations of the procedures you
desire. By doing so, you will be better equipped to make the
surgical decision that is right for you.
Following are some of the most popular facial surgeries:
FACELIFT
 
The
facelift has the most comprehensive effect on your appearance. A
facelift may tighten and rejuvenate the contour of the face,
restoring it to its appearance at a more youthful time.
The facelift offers improvement in the removal of wrinkles and
folds, often times smoothing the lines that form between the nose
and mouth and lifting sagging mid-face skin. A facelift may also
eliminate and/or reduce large jowls that can occur with age,
significantly improving the jaw line. Since the muscles of the neck
can slacken along with those on the face, underlying muscles in this
region may be tightened and excess skin removed as well.
Facelift may be combined with other procedures such as those done on
the eyelid, forehead, or nose. Liposuction may be used to assist
with rejuvenation of the neck.

BLEPHAROPLASTY (EYELID SURGERY)
 
Your eyes reveal more about your
age and emotional state than any other facial feature. As an
example, droopy, puffy eyes and eyelids may create a fatigued, older
appearance than your age. Blepharoplasty, commonly referred to as
cosmetic eyelid surgery, can restore the skin around the eyes to a
more youthful state as well as reduce the signs of aging. When your
upper lids have sagged, the surgery may even serve to improve
impaired vision. Blepharoplasty may be combined with other
surgeries, such as a facelift.

BROWLIFT/
FOREHEAD LIFT
 
When
horizontal lines on the forehead and creases above the nose occur as
part of the aging process, you may look tired, sad, angry and older
than your chronological years. This is when you may opt to have a
browlift, often times done with a facelift. Dr. Lichten may make
discreet, often unnoticeable incisions in the scalp and hairline
where skin can be raised and tightened. Dr. Lichten may also be able
to complete this surgery with the use of an endoscope (fiberoptically
illuminated video camera) to minimize incisions. Muscles can be
repositioned and the brow will heal in this new position. In both
cases, a more awake, younger appearance, may occur.

RHINOPLASTY (NOSE RESHAPING)

If you are unhappy with the size and/or the shape of your nose, you
may become dissatisfied with the way your entire face looks.
Rhinoplasty can adjust the shape of your nose (including the tip
and/or “hump”) and the tilt of the nose to better fit your face. The
procedure can also correct a crooked nose and the breathing
abnormalities that arise due to problems with the internal
structures of your nose.

FACIAL
IMPLANTS
 
Facial
implants, most often to the cheeks and chin, alter the basic shape
and balance of the face. Implants can serve to build a receding
chin, add prominence to cheekbones or reshape the jawline. Implants
may improve the overall proportions of facial features, boosting
self-esteem and making one appear more youthful. At Central Ohio

OTOPLASTY (PROTRUDING EAR SURGERY)
 
When a
child or adult has especially large or protruding ears,
self-confidence can suffer and teasing can occur. It is not unusual
to hear a child refer to another with large ears as, “Dumbo.” Ear
pinning, surgically referred to as otoplasty, can change the size
and shape of the ears. Most often performed between the ages of five
and fifteen, the surgery can be performed on adults as well. Ear
pinning is a highly effective procedure that may improve not only
appearance, but self-esteem too. It is usually a relatively fast and
safe procedure.
Dr.
Lichten also can perform surgery to correct torn or drooping
earlobes. This can usually be done as an office based
procedure any you can return to work the same day.

BOTOX®
 
Botox®
cosmetic is the purified protein form of botulinim toxin. It is a
safe and effective treatment for the temporary reduction of lines
and wrinkles in areas such as the forehead, crow’s feet and between
the brows. Essentially, Botox®
injections paralyze the muscle activity that causes wrinkles to
develop. Additionally, Botox®
has also been shown to be an effective treatment for excessive
underarm perspiration, muscle contractions and migraine headaches.
Treatments may be performed in the office and you can return to work
the same day. The effects are temporary and Botox®
injections typically need to be repeated every 3 to 6 months.

INJECTABLE FILLERS
 
Injectable fillers are used to fill in wrinkles or creases that
develop as the face ages. This produces smoother, more youthful
appearing skin. Fillers may also be used to add fullness to the lips
and cheeks. The results may be temporary and, depending on the
filler used, may need to be repeated every 3 months to 2 years.
Dr. Lichten offers a variety of facial injection techniques to
patients who wish to reduce the appearance of fine lines and/or to
enhance their lips. Fine lines around the nose, mouth, lips and in
the cheeks and chin are gently smoothed with
Restylane® which
typically lasts six months.

COLLAGEN
Collagen
is the fibrous protein that gives the skin its structure and has
been used for decades to replenish the skin’s volume loss. Collagen
is generally safe to smooth out facial lines, wrinkles and scars as
well as to plump pouty lips. The effects of collagen injections
usually last from four to six weeks and can be repeated as
necessary. Skin testing may be necessary as allergic reactions are
possible. Although Collagen has largely been replaced with fillers like Restalyne®,
it is still used in some people.

FAT
Because fat from
one area of a patient’s body, usually from the buttocks and thigh,
is extracted to fill another, there is no chance of allergy of
incompatibility. Fat injections can be used to fill in wrinkles and
lines, create contours in the face, enhance lips and fill in scars.
While usually long lasting, fat injections are not permanent and the
results may be unpredictable.

RECONSTRUCTIVE SCAR REVISION

Many
factors determine the severity of scarring. Some of the most
significant include the size of the wound, its depth, blood supply
to the area, the skin tone and the way the body heals. Scars may
take 18 months to 2 years to fully mature and fade. If after this
time, you are still dissatisfied with the appearance of a scar, you
may consider surgical revision. No surgeon can completely remove a
scar, but often the appearance of a scar can be improved. Scar
revision is a surgical procedure specifically used to improve the
appearance of scars that have not responded to medical modalities
such as cortisone drugs, steroids or skin gels. Several surgical
techniques can be utilized; schedule a consultation to discuss which
surgical option may be right for you.

POST
SKIN CANCER RECONSTRUCTION
 
After
skin cancer has been diagnosed and removed, reconstructive surgery
is often necessary to close or cover the treated area and to restore
the natural appearance of the skin involved in the cancer excision.
The face is often the target of such surgeries. Dr. Lichten may
perform your skin lesion resection in conjunction with the immediate
reconstruction, or you may be referred to or sent from another
surgeon who performs the cancer resection. Dr. Lichten will then
utilize your body’s tissues to close the wound and provide the most
aesthetically pleasing result for your specific situation.

CHEMICAL
PEELS
 
Chemical Peels are an
office based procedure in which a chemical, such as a mild acid, is
applied to the skin to create a controlled peeling of the outer
layer of the skin. This is an excellent option to improve
facial wrinkles and the overall appearance of the skin. At

Patients usually need to pre-treat their skin with Retin-A for 2
months prior to chemical peel and you must not use Acutane for two
years before a chemical peel.

LASER
TREATMENTS
 
At Central Ohio Plastic
Surgery, we use lasers for tattoo removal, the treatment of small
visible blood vessels in the face and legs, hair removal, and facial
resurfacing.
Lasers may also be used to remove the outer layer of skin for
rejuvenation. We use a variety of different lasers, each with a specific wavelength that corresponds to the
intended target. Laser treatments may require more than one
session to achieve the desired result. Pretreatment with Retin-A
may help prepare the skin and Acutane must be avoided for two years
before the procedure.
